Privacy Policy & Notice of Privacy Practices

Two documents on one page. The first explains how this website collects and uses information. The second is our HIPAA Notice of Privacy Practices, which covers your dental records as a patient.

Website Privacy Policy

Who we are

MSD Dental Studio is a dental practice at 1240 N University Dr, Coral Springs, FL 33071, owned by Dr. Matthew Dillon and Dr. Shantall Di Loreto. This policy covers our website, msddentalstudio.com, including its Spanish pages. You can reach us about anything in it by phone or text at 754-240-4820 or by email at admin@msddentalstudio.com.

Two kinds of information, two policies

Information about you as a patient — your health history, X-rays, treatment, insurance and billing — is protected health information under HIPAA. We handle it according to our Notice of Privacy Practices, below. That information lives in our practice-management system, not on this website, and this website has no patient portal.

This section is about the website: what it collects when you visit, read, or send us a message, and what we do with it.

What this website collects

Information you choose to send us

  • Contact forms. Our forms ask for your name, phone number, email, and a message or how you prefer to be contacted. Forms are delivered to us by Formspree, a form-processing service. We read and respond to these messages by phone, text, or email. Please don’t include health details in a web form — web forms are not a secure medical channel. Call or text us instead, or tell us at your visit.
  • Calls and text messages. When you call or text 754-240-4820, we keep your name and number so we can reply and remind you about appointments. If you give us a mobile number, we may text you about scheduling; reply STOP to stop. Message and data rates may apply.
  • Online scheduling. The “Schedule a visit” buttons take you to PatientsReach, the scheduling service we use. Anything you enter there is handled under PatientsReach’s privacy policy as well as our Notice of Privacy Practices once you become a patient.

Information collected automatically

  • Hosting logs. Our site is hosted by Netlify. Like any web server, it records your IP address, browser type, the pages you request, and the time of each request. We use these logs only to keep the site running and secure.
  • Analytics. We use Google Analytics to understand how people use the site — which pages are read, how visitors arrive, and what devices they use. Google Analytics sets cookies and collects your IP address (which Google truncates), device information, and page views. We do not add your name or contact details to analytics data.
  • Advertising measurement. We use the Meta Pixel (Facebook and Instagram) to measure whether our ads lead people to the site and to show our ads to people who have visited it. The pixel sets cookies and sends Meta your IP address, browser information, and the pages you view; if you are logged into a Meta account, Meta can connect that activity to your account under Meta’s privacy policy.
  • Fonts and maps. Our fonts load from Google Fonts and our office map is embedded from Google Maps, so Google receives your IP address when those load. Videos on the site are hosted by us, not by a video platform.
  • Reviews count. Our homepage shows our Google review count. That request goes from our server to Google and does not send Google anything about you.
  • Your cookie choice. When you dismiss the cookie notice, we store that choice in your browser so the notice doesn’t return.

How we use it

  • To answer your message, schedule your visit, and remind you about appointments.
  • To keep the website working, safe, and fast.
  • To understand which pages are useful and improve them.
  • To measure our advertising and reach people who have shown interest in our practice.

We do not sell your personal information, and we do not share it with anyone except the service providers named above (who may use it only to provide their service to us), or when the law requires it.

Cookies and your choices

Our site uses two kinds of cookies: ones that make the site work (remembering your cookie choice and your accessibility settings), and ones set by Google Analytics and the Meta Pixel for measurement and advertising. Our notice on your first visit tells you this. You can limit tracking at any time:

  • Block or delete cookies in your browser’s settings. The site will still work.
  • Install Google’s Analytics opt-out browser add-on.
  • Adjust your Meta ad preferences, including “Activity off Meta technologies,” in your Facebook or Instagram settings.

Our website does not currently respond to “Do Not Track” browser signals, because there is no agreed standard for how to honor them.

Children

This website is for adults and for parents and guardians. It is not directed to children under 13, and we do not knowingly collect personal information from them online.

Your rights

You may ask us what information you have submitted through the website, ask us to correct it, or ask us to delete it. Email admin@msddentalstudio.com and we will respond within 30 days. Requests about your dental records are handled under the Notice of Privacy Practices below, which gives you the right to see and get a copy of your records. Depending on where you live, you may have additional rights under your state’s privacy law; we will honor requests we are required to honor and will tell you if we cannot.

Security

The site is served over HTTPS, so what you send through it is encrypted in transit. No website is perfectly secure, which is one more reason we ask you not to put health details in a web form.

Links to other sites

Our articles link to sources such as the American Dental Association and the National Institutes of Health, and our buttons link to PatientsReach, Google Maps, Facebook, and Instagram. Those sites have their own privacy policies, and this one does not cover them.

Changes to this policy

If we change what the website collects or how we use it, we will update this page and the effective date at the top.

Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your rights

When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

  • Get a copy of your dental record. You can ask to see or get a copy of your dental record and other health information we have about you. Ask us how to do this. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
  • Ask us to correct your record. You can ask us to correct health information about you that you think is incorrect or incomplete. We may say “no” to your request, but we will tell you why in writing within 60 days.
  • Request confidential communications. You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests.
  • Ask us to limit what we use or share. You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care. If you pay for a service or health care item out of pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
  • Get a list of those with whom we have shared information. You can ask for a list (accounting) of the times we have shared your health information for six years prior to the date you ask, who we shared it with, and why. We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We will provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another within 12 months.
  • Get a copy of this privacy notice. You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
  • Choose someone to act for you. If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority and can act for you before we take any action.
  • File a complaint if you feel your rights are violated. You can complain if you feel we have violated your rights by contacting us using the information at the end of this notice. You can also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/. We will not retaliate against you for filing a complaint.

Your choices

For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.

In these cases, you have both the right and choice to tell us to:

  • Share information with your family, close friends, or others involved in your care.
  • Share information in a disaster relief situation.

If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.

In these cases we never share your information unless you give us written permission:

  • Marketing purposes.
  • Sale of your information.
  • Most sharing of psychotherapy notes.

In the case of fundraising: we may contact you for fundraising efforts, but you can tell us not to contact you again.

Our uses and disclosures

How do we typically use or share your health information? We typically use or share your health information in the following ways.

  • Treat you. We can use your health information and share it with other professionals who are treating you. Example: a dentist treating you asks another specialist about your overall health condition.
  • Run our organization. We can use and share your health information to run our practice, improve your care, and contact you when necessary. Example: we use health information about you to manage your treatment and services.
  • Bill for your services. We can use and share your health information to bill and get payment from health plans or other entities. Example: we give information about you to your dental insurance plan so it will pay for your services.

How else can we use or share your health information? We are allowed or required to share your information in other ways — usually in ways that contribute to the public good, such as public health and research. We have to meet many conditions in the law before we can share your information for these purposes. For more information see www.hhs.gov/hipaa/for-individuals.

  • Help with public health and safety issues. We can share health information about you for certain situations such as preventing disease; helping with product recalls; reporting adverse reactions to medications; reporting suspected abuse, neglect, or domestic violence; and preventing or reducing a serious threat to anyone’s health or safety.
  • Do research. We can use or share your information for health research.
  • Comply with the law. We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we are complying with federal privacy law.
  • Respond to organ and tissue donation requests. We can share health information about you with organ procurement organizations.
  • Work with a medical examiner or funeral director. We can share health information with a coroner, medical examiner, or funeral director when an individual dies.
  • Address workers’ compensation, law enforcement, and other government requests. We can use or share health information about you for workers’ compensation claims; for law enforcement purposes or with a law enforcement official; with health oversight agencies for activities authorized by law; and for special government functions such as military, national security, and presidential protective services.
  • Respond to lawsuits and legal actions. We can share health information about you in response to a court or administrative order, or in response to a subpoena.

Our responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.

Changes to the terms of this notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on this page.
